FOLLOW ONE OF THE STEPS LISTED BELOW TO OBTAIN YOUR TRANSCRIPTS. TRANSCRIPTS WILL BE MAILED WITHIN 48 HOURS OF RECEIVING THE REQUEST.
1. Come into Parker High School Guidance Office and sign a Permission to Release Records form. Parker Guidance Office hours are 7:30 am – 4 pm.
2. If you no longer live in the Janesville area or you can not get into the guidance office you can mail a letter to us with the following information:
a. Your name when you attended Parker High School
b. Your Date of Birth
c. Graduation Year or Year of Withdrawal
d. Where do you want these transcripts mailed to (i.e. another school, place of employment, yourself) Give a complete address
e. SIGN THIS RELEASE
f. Mail the above information along to:
